When administered correctly, it's considered a safe anesthetic option. Yet, medical professionals who frequently administer it should be aware of potential hazards associated with long-term exposure.</p> <p>This article will examine the research and possible side effects of N<sub>2</sub>O exposure, commonly known as laughing gas, in dental settings.</p> <h3><strong>Retrospective Studies</strong></h3> <div class="mobileimage" style="width: 440px; max-width: 50%; float: right; margin-left: 1em; margin-bottom: 1em; text-align: center; padding:4px; border: 1px solid #eee; background: #ffffff;"> <figure><img alt="" data-entity-type="file" data-entity-uuid="5728caa3-05a6-4897-b437-e0d3af749f03" src="//www.docseducation.com/sites/default/files/inline-images/bl_ex_4.jpg" style="width: 100%;" /></figure></div> <p>In 1967, Vaisman(<em><strong>1</strong></em>), a Russian scientist, reported that female and male anesthesiologists experienced reproductive issues at a much higher rate than the general population. Given that N<sub>2</sub>O was a common denominator in the anesthetic applications studied, it was implied as the causative agent.</p> <p>In the 1970s and 80s, <a href="https://med.stanford.edu/news/all-news/2010/06/stanford-anesthesiologist-ellis-cohen-dies-at-91">Ellis N. Cohen, MD</a>, a Stanford University School of Medicine anesthesiologist whose research revolutionized how operating rooms are now designed, published articles(<em><strong>2</strong></em>) on anesthetic health hazards.</p> <p>One of Cohen's studies surveyed over 50,000 dentists and dental assistants exposed to trace anesthetics. The results suggested that long-term exposure to anesthetic gases could lead to general health problems and reproductive difficulty for those exposed to trace anesthetics. The results indicated that long-term exposure to anesthetic gases could be associated with an increase in general health problems and reproductive difficulty.</p> <p>Although research studies have continued to reveal the potentially harmful effects of chronic exposure to N<sub>2</sub>O, it's important to note that these studies have been retrospective. As a result, there may be biases in the study design that could affect the accuracy of the results. Due to these flaws, whether there's a causal relationship between an individual's exposure to N<sub>2</sub>O/O2 and reproductive issues cannot be definitively established.</p> <h3><strong>Dental Setting Exposure</strong></h3> <p>In 1977, the National Institute for Occupational Safety and Health (NIOSH) evaluated the scavenging potential of equipment used in operating rooms and outpatient settings. They found that achieving 25 ppm of N<sub>2</sub>O was possible in the operating room but not in a dental operatory.</p> <p>As a result, NIOSH established a maximum exposure limit of 50 ppm for personnel in a dental setting. Although the Occupational Safety and Health Administration (OSHA) has not strictly enforced this limit, it's still the recommended standard.</p> <p>Despite the negative publicity N<sub>2</sub>O has received, other studies have claimed that no harmful health effects are associated with chronic exposure to N<sub>2</sub>O, especially in low concentrations.</p> <h3><strong>Further Evaluation Indicates Exposure Duration Factor</strong></h3> <p>In 1995, the University of Colorado conducted a worldwide literature search on biohazards associated with N<sub>2</sub>O use. Their literature review indicated no scientific basis for the previously established threshold levels for the hospital operating room or the dental setting. This result led to a meeting of interested parties representing dentistry, government, and manufacturing. A formal position statement stating that a maximum N<sub>2</sub>O exposure limit in parts per million has not (yet) been determined.</p> <p>There is some evidence that chronic exposure to high levels of N<sub>2</sub>O affects reproduction. Still, no evidence has been found to date that a<em><strong> direct causal relationship</strong></em> exists between reproductive health and scavenged low levels of N<sub>2</sub>O.</p> <p>Sweeney et al.(<em><strong>3</strong></em>) conducted a study that showed the first indication of any biological effect from N<sub>2</sub>O was observed at a chronic exposure level of 1800 ppm. Experts agree that 400 ppm is a reasonable exposure level significantly below the established biological threshold. (Sweeney et al.)</p> <p>N<sub>2</sub>O does interfere with the enzyme methionine synthase, which recycles homocysteine back to methionine. Methionine synthase is crucial in the metabolic pathways of vitamin B12 and folic acid and the production of DNA. Therefore, prolonged exposure to N<sub>2</sub>O can lead to megaloblastic hematopoiesis and leucopenia.</p> <p>It's also hypothesized that this interference might impair fetal development. In animal studies, pregnant rats exposed to approximately 60% N<sub>2</sub>O for 24 hours experienced miscarriage and other fetal abnormalities. For pregnant women working in environments where N<sub>2</sub>O is used, it's crucial to know the levels of N<sub>2</sub>O exposure and to use all recommended trace gas scavenging methods.</p> <p>Dental professionals should decide, depending on the individual and the situation, whether to avoid the office setting and any N<sub>2</sub>O exposure during the first trimester.</p> <p>Discussions are ongoing regarding the safety of N<sub>2</sub>O. However, the gas appears safe when administered in low therapeutic doses for short periods. The dental team must take several measures to maintain safe levels of N<sub>2</sub>O in the operatory. This includes:</p> <ul><li>A scavenging system can redirect N<sub>2</sub>O outside the office, which is the most effective method. A scavenging mask system has become standard on all product lines in the US and Canada. When expired, N<sub>2</sub>O is exhaled through the nose. Vacuum suction ports transport the gas through the central suction to the outside atmosphere. NIOSH has established that an evacuation flow rate of 45 L/min is optimal.</li> <li>Good ventilation in the operatory can dilute the concentration of the gas.</li> <li>Well-fitting masks can reduce leakage. One of the newest advances in nitrous oxide administration is the <a href="https://safesedate.info/shop/"><em><strong>Safe Sedate Nasal Hood</strong></em></a>. This apparatus provides more efficient delivery of gases into the nasal nares while reducing extraneous nitrous oxide into the operatory. It is also latex-free and disposable, preventing cross-contamination.</li> <li>Reducing talking by the patient decreases the exhalation of nitrous oxide into the operatory environment.</li> <li>Gas can leak at any connection on the equipment, including the manifold, wall-mounted connections, and associated hoses. Conducting tubing and the reservoir bag are also areas of concern. Because these materials break down over time, they can crack and leak around pleats and seams.</li> <li>The soap and water method for determining leaks is recommended for inspecting these areas. The bubbles after applying the soapy water indicate a gas leak.</li> <li>Various commercially available monitoring systems can alert dental staff to potentially unsafe N<sub>2</sub>O exposure levels. These systems include infrared (IR) spectrophotometry, time-weighted average (TWA) dosimetry, and hand-held monitoring devices.</li> </ul><h3><strong>In Conclusion</strong></h3> <p>No direct evidence shows chronic low-level exposure to N<sub>2</sub>O causes prolonged and permanent biological effects. Yet, the maximum safe concentration of N<sub>2</sub>O isn’t fully understood. That's why all dental team members should take safety measures to reduce the level of trace N<sub>2</sub>O they're exposed to. This breakthrough research highlights not only the interconnectedness of our bodily systems but also opens new avenues for understanding and potentially combating one of the most lethal forms of cancer.</p> <h3><strong>The Intricate World of the Oral Microbiome - A Universe Within Our Mouths</strong></h3> <h4><em>The human mouth is home to a diverse community of over <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3705763/" target="_blank">700 species</a> of bacteria, viruses, fungi, and protozoa, collectively known as the oral microbiome.</em></h4> <p>Far from being mere passengers, these microorganisms play crucial roles in digesting food, protecting against pathogens, and influencing our overall health. The complex ecosystem of oral bacteria has been identified as a significant player in the onset of various periodontal diseases, which, in turn, can spark systemic inflammation and contribute to the emergence of <a href="https://microbiomejournal.biomedcentral.com/articles/10.1186/s40168-022-01262-7" target="_blank">several systemic disorders</a>, including rheumatoid arthritis, cardiovascular diseases, pulmonary diseases, diabetes, and Alzheimer’s disease.</p> <p>Emerging research indicates that the effects of periodontitis extend far beyond the oral cavity. The bacteria responsible for gum disease don't confine their activities to the mouth; they can enter the bloodstream, setting off inflammatory processes throughout the body.</p> <h3><strong>Bridging Oral Microbiomes to Pancreatic Cancer Through Research</strong></h3> <div class="mobileimage" style="width: 450px; max-width: 50%; float: right; margin-left: 1em; margin-bottom: 1em; text-align: center; padding:4px; border: 1px solid #eee; background: #ffffff;"> <figure><img alt="" data-entity-type="file" data-entity-uuid="07e6dcb2-1f1a-4e6b-afe4-35ecee88b8b5" src="//www.docseducation.com/sites/default/files/inline-images/bl_ex_3.jpg" style="width: 100%;" /></figure></div> <p>Recent research has established a strong link between the oral microbiome and the progression of malignant tumors, notably highly aggressive pancreatic cancer. The 5-year survival rate of pancreatic cancer is just <a href="https://microbiomejournal.biomedcentral.com/articles/10.1186/s40168-022-01262-7" target="_blank">11%.</a></p> <p>In a pioneering prospective <a href="https://pubmed.ncbi.nlm.nih.gov/17228001/" target="_blank">cohort study</a> focusing on male health professionals, a significant link was identified between periodontal disease and an increased risk of pancreatic cancer. The findings revealed that men with a history of periodontal disease exhibited a 64% elevated risk of developing pancreatic cancer in comparison to their counterparts without such a history. Interestingly, this risk doubled in men who had never smoked, effectively dismissing smoking as a confounding factor in the relationship between periodontal disease and pancreatic cancer. This correlation aligns with outcomes from other research efforts, which similarly report an association between periodontal health issues—or tooth loss—and a heightened risk of pancreatic cancer, further emphasizing the potential systemic impact of oral health conditions.</p> <p>In another groundbreaking research, the investigations led by Farrell shed light on the complex interplay between oral bacteria and pancreatic cancer risk. <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3705763/" target="_blank">Farrell</a> meticulously cataloged sixteen species and clusters of oral bacteria, including diverse groups such as <em>Streptococcus, Prevotella, Campylobacter, Granulicatella, Atopobium, </em>and<em> Neisseria </em>from the salivary microbiota of individuals diagnosed with pancreatic cancer against that of healthy counterparts. This finding suggests a notable alteration in the oral microbial ecosystem of pancreatic cancer patients, pointing towards potential biomarkers for early detection or risk assessment of the disease.</p> <p>The link between periductal adenocarcinoma (PDAC), one of the most aggressive forms of cancer (with a 5-year survival rate of just <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6250924/">6%</a>), and oral health has become increasingly apparent with recent findings highlighting the involvement of Porphyromonas gingivalis. This anaerobic bacterium, commonly associated with periodontal disease, has been identified as a potential factor contributing to the development of PDAC.</p> <p>Intriguing research has revealed a direct pathway through which P. gingivalis, a bacterium linked to periodontal disease, can influence the development of pancreatic cancer. When applied to the gums of healthy mice, viable P. gingivalis was detected in their pancreas, demonstrating its ability to migrate and alter the organ's microbial equilibrium. Notably, in mice genetically predisposed to pancreatic issues, the introduction of P. gingivalis expedited the transition from minor pancreatic anomalies to full-blown cancer. Further, the study uncovered that a specific genetic mutation not only facilitated the survival of P. gingivalis within cellular environments but also appeared to bolster the resilience of pancreatic cancer cells under adverse conditions.</p> <h4><em>This groundbreaking work underscores the complex interplay between oral health and systemic diseases, particularly highlighting the impact of oral pathogens on pancreatic health.</em></h4> <p>In a complementary study, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3815505/" target="_blank">Michaud</a> focused on 25 preselected oral bacteria species, with a keen interest in <em>Porphyromonas gingivalis </em>and<em> Aggregatibacter actinomycetemcomitans</em>, revealing that the immune response to oral pathogens can have a significant impact on the risk of pancreatic cancer.</p> <p>Specifically, the team discovered that individuals harboring antibody levels greater than 200 ng/ml against the periodontal bacterium P. gingivalis experienced a twofold increase in pancreatic cancer risk compared to those with lower antibody concentrations (≤200 ng/ml). Identifying specific microbial signatures associated with pancreatic cancer risk could lead to early detection methods or preventive interventions. Furthermore, manipulating the oral microbiome could become a component of personalized treatment strategies.</p> <p>Regular dental check-ups and maintaining good oral hygiene may emerge as simple yet effective strategies for reducing pancreatic cancer risk.</p> <p>Understanding the link between oral bacteria and pancreatic cancer also offers exciting possibilities for developing new therapeutic approaches. Understanding how these drugs interact with sedation medications is critical to patient safety.</p> <p>In this course, DOCS Education Medical Director Leslie Fang, MD, PhD, discusses drug metabolism mechanisms in the liver and intestines. Dr. Fang breaks down the critical nature of the P450 (CYP) enzyme system responsible for the metabolism of many drugs to help you recognize how medications, including those used for sedation, work together and against each other.</p> <p>Primary objectives of this course include:</p> <ul><li>Understanding the role of the P450 (CYP) in drug metabolism.</li> <li>Recognizing the critical actions of the metabolism of drugs.</li> <li>Learning how certain supplements and juices can affect the CYP3A4 enzymes.</li> <li>Identifying which drug and delivery system of a drug with C interactions isn’t the best choice.</li> <li>Determining which types of interactions are concerning and which are not.</li> </ul><h3><strong>Drug Metabolism</strong></h3> <p>While the liver is primarily responsible for drug metabolism, it’s now understood that the intestines also play an essential role.</p> <p>Regarding sedation drugs, the enzyme system responsible for the metabolism of most of those drugs used for conscious sedation belongs to the Cytochrome P450 system, particularly CYP3A4. P450 is going to be (90%) found in the liver, but there appears to be CYP3A4 in the mucosa of the small intestine.</p> <h3><strong>Drug-Drug Interactions</strong></h3> <div class="mobileimage" style="width: 420px; max-width: 50%; float: right; margin-left: 1em; margin-bottom: 1em; text-align: center; padding:4px; border: 1px solid #eee; background: #ffffff;"> <figure><img alt="" data-entity-type="file" data-entity-uuid="c966e639-048e-4232-a49f-b4229a0aa9e4" src="//www.docseducation.com/sites/default/files/inline-images/bl_ex_2.jpg" style="width: 100%;" /></figure></div> <p>Whenever we talk about enzymatic metabolism by CPY3A4, triazolam will fit into the active site like a key into a lock and undergo a simple metabolic breakdown. Therefore, anything that interferes with that reaction is going to constitute drug-drug interaction. Two interactions to be aware of are:</p> <ul><li><strong>C interaction</strong>: Minor may choose to sedate over it.</li> <li><strong>D Interaction</strong>: Major-requires modification.</li> </ul><p>For example, Dilantin (phenytoin) and barbiturates are D interactions. These drugs are potent inducers of CYP3A4 and are primarily used for seizure disorders. Sedation dentists must be aware that these drugs can have multiple drug-drug interactions. Therefore, clinical use of these particular medications is decreasing due to concerns about drug-drug interactions.</p> <p>Because of the D interaction of these drugs, patients on these medications will require modification to their sedation protocol. Furthermore, it's critical that patients on these drugs do not stop them, or they can seize. When sedating these patients, use medications not mediated through CYP3A4, such as Lorazepam.</p> <p>This is further evidence of why patients MUST report ALL prescription and non-prescription, including all nutraceuticals and supplements taken, including the likes of St. John’s wart (C interaction), which can interfere with sedation.</p> <h3><strong>Inhibitors</strong></h3> <p>Agents known to inhibit CYP3A4 include (D interactions) such as:</p> <ul><li>Grapefruit juice, also called a “suicide inhibitor,” inhibits the CYP3A4 in the intestines but not the hepatic enzymes. Therefore, <strong>be sure to ask patients about consuming grapefruit juice</strong>.</li> <li>Diltiazem (Cardizem)</li> <li>Verapamil (Calan, Isoptin SR)</li> <li>Clarithromycin (Biaxin)</li> <li>Fluconazole (Diflucan)</li> <li>Itraconazole (Sporanox)</li> <li>Nefazodone (Serzone)</li> </ul><h3><strong>Possible Alternatives</strong></h3> <p>While C interactions don’t cause the same inhibitions as D interactions, there are better ways around these drugs. Some of these medications include:</p> <ul><li>Omeprazole (Prilosec)</li> <li>Esomeprazole (Nexium)</li> <li>Fluoxetine (Prozac)</li> </ul><p>For all the drugs with C interactions, you can sedate over them or find a way around the interactions. Patients can become deeply attached to your team when they have great experiences with the same familiar faces year after year. With a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4949805/" target="_blank">cohesive team</a> that supports each other in delivering exceptional care, your patients will stay with you and recommend your team to their friends and loved ones.</p> <p>Let's review five easy ways to improve connections between your team members so your practice can thrive.</p> <h3>1. Knowing this might put them behind schedule, team members can make themselves available to flip the operatory while the patient is being helped to the front desk.</p> <p>It’s also a chance to share celebrations. Is a patient coming for the final seat on their full mouth rehab tomorrow? Be sure there’s a congratulations card waiting for them!</p> <p>The morning huddle allows you to help everyone work as a unit and stay mindful of each other’s needs. This will help everything flow, give your patients the best possible experience, and build goodwill between team members.</p> <p><strong>WHAT NOT TO DO:</strong> The morning huddle is not the time for a general airing of grievances, and it’s not the best time to troubleshoot deep or complex issues. If there’s a quick fix that needs attention (e.g., “Quick reminder to check the patient bathroom for toilet paper at least once around lunchtime.”), that’s fine. But don’t let it become a tit-for-tat blame session. Schedule separate opportunities to work out those kinds of issues.</p> <h3>2. Consistent Systems</h3> <p>Consistent, predictable systems take out the guesswork and keep everyone working on the same page.</p> <p>Systems should be simple, efficient, and easy to follow long term. Systems are especially important in sterilization and restocking, two back-of-house areas where friction starts.</p> <p>Established routines decrease frustrations about uneven workloads because everyone follows the same steps to keep things running smoothly. And if they’re not, it’s easy to identify a specific area to improve.</p> <p>Predictability also means your team spends less mental energy thinking about each step that needs to be done because it becomes muscle memory after a while. That will help them do their jobs more efficiently and prevent errors like the autoclave not being tested or maintained on time.</p> <p><strong>WHAT NOT TO DO: </strong>Being too rigid about the routine means you’ll miss out on adjustments that could make things even better. Consistent systems are great if you allow them to adapt and evolve.</p> <h3>3. Open Communication</h3> <p>Each team member should have access to a written statement of your philosophy of care and mission as a practice. This will start new team members on the right foot and give established staff a touchstone to keep them focused on your highest values as healthcare providers.</p> <p>Other than the morning huddle, make time to <a href="https://www.ada.org/resources/practice/practice-management/managing-effective-team-meetings" target="_blank">check in with your team regularly</a>. Schedule a standing practice-wide meeting at least once every month. Your team might also benefit from a focused, quick monthly meeting <a href="https://www.rdhmag.com/career-profession/inspiration/article/14301887/a-cohesive-team-is-a-valuable-team-tips-to-get-on-the-same-page" target="_blank">within each department</a>, too.</p> <p>Private quarterly chats with each team member will allow you to praise great performance and nip toxic behaviors in the bud.</p> <p>Listen objectively to concerns about what’s not going smoothly. Try to work as a group to find solutions that work for everyone.</p> <p><strong>WHAT NOT TO DO:</strong> Try to avoid scheduling meetings only when things are going wrong. If open communication is always a negative experience, your team will dread it rather than see it as an opportunity to connect. Check in with staff when things are going well, too!</p> <h3>4. Grow Together</h3> <p>Offer your team opportunities to expand or deepen their practice as a group. That could mean attending dental conferences or CE classes together or scheduling regular lunch-and-learns.</p> <p>Those monthly staff meetings are also a great time to allow each team member to share insights. Did one of your hygienists devise a great way to help a patient cope with dental phobia? (1) The NIH also estimates that the same age group has approximately one missing or decayed permanent tooth per child.</p> <p>While these numbers have decreased since the NIH’s 2004 survey, the number of children with dental caries and tooth decay still requires significant improvement.</p> <h3><strong>Oral Health Impacts on Learning</strong></h3> <div class="mobileimage" style="width: 480px; max-width: 50%; float: right; margin-left: 1em; margin-bottom: 1em; text-align: center; padding:4px; border: 1px solid #eee; background: #ffffff;"> <figure><img alt="" data-entity-type="file" data-entity-uuid="aed16f5f-940a-4a34-992a-a17fa186c10f" src="//www.docseducation.com/sites/default/files/inline-images/bl-ex-4_33.jpg" style="width: 100%;" /></figure></div> <p>Dental caries are one of the most common chronic childhood diseases and often result in pain and missed school days. Untreated cavities can also lead to more severe complications such as infection and abscesses.</p> <p>In addition, cavities can significantly affect a child's ability to concentrate, affecting their academic performance and creating a perpetual cycle of oral health issues and negative educational impact.</p> <p>As a result, the Centers for Disease Control and Prevention (CDC) recommends school sealant programs to mainly target children who are less likely to visit a dentist.</p> <h3><strong>Is SDF Cost-Effective?</strong></h3> <p><a href="https://jamanetwork.com/journals/jamapediatrics/fullarticle/2815515" target="_blank">Researchers at NYU College of Dentistry</a> have discovered an inexpensive liquid to fight dental carries called silver diamine fluoride (SDF). Their study followed more than 4,000 elementary school students for four years and showed SDF as an effective, cheaper alternative to dental sealants while also increasing access to oral healthcare.</p> <p><a href="https://dental.nyu.edu/faculty/ft/rn45.html" target="_blank">Richard Niederman, DMD</a>, professor of epidemiology and health promotion at NYU College of Dentistry and the study’s senior author, is quoted in NYU’s press release as stating, “A growing body of research shows that SDF—which is quicker to apply and less expensive than sealants—can prevent and arrest cavities, reducing the need for drilling and filling.”</p> <h3><strong>SDF vs. Traditional Sealants</strong></h3> <p>NYU College of Dentistry researchers piloted <em><strong>CariedAway™ </strong></em>(developed by NYU Dentistry and supported by Northeast Delta Dental), the most comprehensive school-based cavity prevention program, to assess the effectiveness of SDF and traditional sealants.</p> <p>The study was conducted in elementary schools in New York City and involved approximately 4,100 children, of whom a significant percentage (over 25%) had untreated cavities at the beginning. The study compared SDF and traditional sealants to evaluate their impact on cavity prevention.</p> <p>Depending on whether the school had been randomly assigned sealants or SDF followed by fluoride varnish, a team of health professionals examined and treated children's teeth. Dental hygienists applied sealants, while registered nurses applied SDF, all under the supervision of a dentist. In 2018, the team visited each school twice a year, but school closures and the COVID-19 pandemic caused some visits to be missed.</p> <p>In the journal <em>JAMA Network Open</em>, <a href="https://www.nyu.edu/about/news-publications/news/2023/february/school-dental-program-prevents-80-percent-of-cavities.html" target="_blank">researchers reported last year</a> that a single treatment of either SDF or sealants prevented 80% of dental carries and 50% from worsening two years later.</p> <p>In their study published in <em>JAMA Pediatrics</em> (2), the team concluded that after following children for four years, SDF and sealants prevented roughly the same number of dental caries. Furthermore, due to SDF's cost-effectiveness, it can prove effective as an option (not a replacement) to dental sealants in halting tooth decay. This study also shows that sealants are equally effective in treating tooth decay and vital to school-based cavity prevention programs.</p> <p>Tamarinda Barry Godín, DDS, MPH, associate program director and supervising dentist for <strong><em>CariedAway</em></strong><em><strong>™</strong></em>, research scientist at NYU College of Dentistry, and the study’s coauthor, is quoted in NYU’s press release as stating, “Most research shows that SDF can stop a cavity from progressing further. Monitoring should include oxygenation, ventilation, circulation, and temperature. Patient consciousness, however, must always be prioritized, as it's the foundation of their well-being and safety throughout the procedure.</p> <p>Three critical factors must be considered to ensure patient safety. Let's explore these elements to ensure your patient's safety is never compromised. They include:</p> <ol><li>Patient selection.</li> <li>Avoid sedating the very young, the very old, and the very sick.</li> <li>Keep your patient conscious. (DOCS only teaches conscious sedation)</li> </ol><p>Risks inherently grow as your patient becomes increasingly sedated. It's imperative to monitor vital statistics and the patient’s airway, as well as support the natural characteristics that help them breathe and maintain their life functions. Maintaining verbal responsiveness with your patient throughout the procedure should be a top priority.</p> <h3>Goals of Monitoring</h3> <div class="mobileimage" style="width: 480px; max-width: 50%; float: right; margin-left: 1em; margin-bottom: 1em; text-align: center; padding:4px; border: 1px solid #eee; background: #ffffff;"> <figure><img alt="" data-entity-type="file" data-entity-uuid="6b2031cd-f160-49f4-b392-26c788f4d418" src="//www.docseducation.com/sites/default/files/inline-images/bl-ex-3_30.jpg" style="width: 100%;" /></figure></div> <ul><li>Observe consciousness</li> <li>Verbal responsiveness</li> <li>Monitor physiological parameters</li> <li>Heart rate</li> <li>Oxygen saturation</li> <li>Blood pressure</li> <li>End-tidal carbon dioxide (ETCO2)</li> </ul><p>When monitoring a patient during sedation, focusing on the machine that measures vital statistics is common. However, the most crucial factor to keep track of is the patient's consciousness, which can only be done by direct interaction with the patient rather than relying solely on the readings from monitors.</p> <h3>Pre-Sedation Baselines</h3> <p>The patient's "normal" BP range should fall within the ASA 1 and ASA 2 categories.</p> <ul><li>Preferred heart rate of: &gt;60 and &lt;100</li> <li>Preferred blood pressure of: &lt;120 systolic and &lt;80 diastolic</li> <li>Preferred 02 saturation of: &gt; 96%</li> <li>Preferred ETCO2 of: 40mm HG +/- 5 mm</li> </ul><p>When the preoperative heart rate and blood pressure are above these preferred levels, the patient’s anxiety is often contributing. Sometimes, taking readings without the doctor present can provide a more reliable and consistent measurement of the patient.</p> <p>Lower-than-normal heart rate and blood pressure readings are often found in patients in excellent physical condition. When abnormal baseline cardiovascular values cannot be explained by anxiety or excellent physical condition alone, patients should seek medical consultation before sedating them.</p> <p>As with any atypical measurement, the inaccuracy of the monitoring equipment must also be ruled out. Key points to remember include:</p> <ul><li>Do not sedate without high-quality monitoring equipment. It’s also essential that all team members know how to use the equipment and how to set the alarms.</li> <li>Pulse, blood pressure, oxygen saturation, and end-tidal CO2 measurements must be measured, monitored, and recorded throughout the treatment.</li> </ul><h3>Monitoring Heart Rate</h3> <p>When a patient is sedated, their heart rate may slow down due to the activation of the parasympathetic system, the release of acetylcholine, and the activation of its receptors.</p> <p>This decrease in heart rate is to be expected and less of a concern compared to tachycardia, which increases the risk of an adverse cardiovascular event. Additionally, there is particular concern when an absence of a P wave on an EKG accompanies the tachycardia, which can be a sign of heart block, ventricular tachycardia, or fibrillation.</p> <p>Certain medical conditions can affect the patient’s heart rate. Examples of ailments that can increase heart rate include:</p> <ul><li>Atherosclerosis</li> <li>Congestive Heart Failure</li> <li>Myocardial Infarction</li> <li>Congenital Heart Defects</li> <li>Inflammatory or Degenerative Heart Disease</li> <li>Myocarditis (Infection of the Heart)</li> <li>Chronic Lung Disease</li> <li>Hypertension</li> <li>Hyperthyroidism</li> <li>Electrolyte Imbalance</li> <li>Emotional Stress</li> </ul><p>Medications that can cause tachycardia include:</p> <ul><li>Asthma Medicines</li> <li>Antibiotics</li> <li>Cough, Cold, and Allergy Medicines</li> <li>Thyroid Medicine</li> <li>Antidepressants</li> <li>Supplements</li> </ul><h3>Monitoring Blood Pressure</h3> <p>Patients diagnosed with regular or prehypertension are acceptable candidates for sedation. Given all other health conditions the patient might have, it’s acceptable for a patient with stage 1 hypertension also to be sedated, provided the consult with their PCP or cardiologist determines their hypertension to be under acceptable control.</p> <p>However, patients with stage 2 hypertension are not candidates for sedation until their condition is successfully managed.</p> <h3>Abnormal Blood Pressure During Sedation</h3> <p>The blood pressure of a sedated patient should be lower than their baseline blood pressure, but significant differences may still occur. This is due to the following reasons:</p> <ul><li>Sedation medications</li> <li>Existing medications</li> <li>Skipped or missed medication dose</li> <li>Patient discomfort</li> <li>Indication of a possible cardiovascular event</li> <li>Monitoring equipment</li> </ul><p>It’s important to remember the blood pressure cuff size you’re using for each patient. For instance, if the cuff is too small for the patient, the BP reading will be artificially high. Conversely, if the cuff is too large for the patient, the BP reading will be artificially low. If the BP is taken on an extremity above the level of the heart, the reading will also be artificially low.</p> <h3>Monitoring Oxygenation</h3> <p>Oxygenation is the most critical factor among the various physiological measurements assessed during conscious sedation. While heart rate and blood pressure are also monitored, they are typically supported by the sedation process, making them less critical than oxygenation.</p> <p>Although end-tidal CO2 measures ventilation, oxygenation is a more revealing number in conscious sedation patients. Since they are not intubated, capnography/capnometry measurement reliability is less pertinent.
